By submitting this form, I understand and agree that the relationship being entered into is one of volunteerism and not employment. There will be no payment or fringe benefits that may be enjoyed by employees of Church Without Walls New Orleans, and that either party may terminate the volunteer services at any time, without cause and without prior notice.

I understand that filling out this form does not register me to volunteer at any of the Church Without Walls' events. Upon completing the volunteer orientation, I will call the volunteer department to schecdule a time to serve.
